Our Health Care Support service has been designed to provide assistance for people who may need help and support after returning home after a period in hospital or residential care a period of illness, or the onset of a disability.

Our Health Care support service helps to regain the skills for independent living, learning new skills or to allow the individual to deal with their changed abilities.
Individuals have the opportunity to regain confidence to look after themselves in their own homes, with professional staff that assess and carry out a therapeutic plan, This plan is followed at the customers’ own pace with time given for each individual to recover their strength and or to come to terms with their changed condition. Each individual is fully involved in the preparation of this plan.

The service is provided to people within their own homes for a maximum of 6 weeks and is regularly reviewed to monitor progress.

The projected outcomes of the Health Care Support team are:

• Highest quality of life for the customer
• To assist people to achieve maximum independence and to remain at home for longer
• To avoid unnecessary hospital admissions and re-admissions
• To reduce service users ongoing dependency on community based service.
• To maintain and increase abilities in supporting themselves.

The service is provided by a dedicated core team of home care workers who have been trained in Reablement skills. This ensures that where appropriate, a well orientated support plan is followed and the highest level of care is maintained.
The customer is empowered to achieve a graduated return to the maximum levels of independence.

One of the benefits of the service is that information from the Reablement session can feed into the assessment that determines ongoing support, thus enabling more than a snapshot of an individuals needs to be built.

Wherever possible the team involves relevant members of a customers’ family to ensure that carers needs are met.
